Transcript Request

How to Request a Transcript or Record of Grade 

  1. Download the Transcript Request Form HERE.
  2. When you have filled the Transcript Request form out completely, submit the form to the SoCal School of Ministry.  Transcripts can be submitted in any of the following ways: E-mail, Mail, Fax, In Person.
  3. If you are requesting a Course Grade report, pay the $5 fee HERE.
  4. If you are requesting an Official Transcript, pay the $15 fee HERE.

*Please note that Transcripts will not be sent until the student has paid for their classes in full & the Transcript Request Fee has been paid.  Due to the volume of transcript requests that we receive, and our dedication to the timely processing of those requests, our typical processing time is 1-5 business days.
